What information we may collect

Depending on the services you receive, we may collect contact and identity details, NDIS and service information, goals and preferences, funding and billing information, records of communications, and health, disability or other sensitive information that is reasonably necessary to provide your supports. We aim to collect sensitive information only when it is needed and with consent, unless collection is otherwise required or authorised by law.

How we collect information

Information may be collected directly from you by phone, email, in person or through forms and documents you provide. With your consent, we may also receive relevant information from a nominee, family member, plan manager, support provider, health professional, the NDIA or another person or organisation involved in your supports.

Why we use information

We use information to respond to enquiries, assess whether our service is suitable, provide and coordinate agreed supports, communicate with people you authorise, monitor service arrangements and budgets, keep appropriate records, process invoices, respond to feedback or complaints, manage quality and safety, and meet legal or regulatory obligations.

Sharing information

With your permission, relevant information may be shared with people or organisations involved in your supports, such as chosen providers, allied health professionals, a plan manager, nominee or advocate. Information may also be disclosed without consent where required or authorised by law, including where necessary to respond to a serious threat to life, health or safety.

How information is kept

Reasonable steps are taken to protect personal information from misuse, interference, loss and unauthorised access, modification or disclosure. Access to participant information is limited to people who need it for their role, and records are retained and securely disposed of in accordance with applicable legal and record-keeping requirements.

Website enquiries

This website currently uses direct phone and email links rather than a web form. Please avoid sending your full NDIS plan, NDIS number or detailed medical information in an initial email. If documents are needed, an appropriate way to share them can be discussed first.

Access, correction and consent

You can ask to access or correct personal information held about you. You can also discuss or change who we are permitted to share information with, and withdraw or amend consent where the law allows. A full Privacy and Information Management Policy and Privacy Consent Form, including an Easy Read version, are available on request.

Privacy complaints

If you have a privacy concern, please contact us first if you feel comfortable doing so. We will take the concern seriously and handle it through our complaints and incident processes. You may also have the right to contact the Office of the Australian Information Commissioner ↗ or, for health information handled in Victoria, the Health Complaints Commissioner ↗.
